REQUESTING MADD'S EDUCATIONAL SERVICES IN NORTHERN VIRGINIA

This page explains how to request our Crashed Car Trailer Exhibit, a MADD Speaker or a MADD Booth Display. "Request Forms" for MADD's educational services will follow this page. After you fill in the appropriate form, PLEASE FAX the Crashed Car Trailer Exhibit form to our office at 1-804-353-7122 or FAX the Speaker or Booth Display form to our office at 703-379-1930.

Please be assured that we will put forth all possible effort to fill your request; keep in mind, however, that these forms are only a request, and do not guarantee MADD's attendance. Requests are always confirmed by our office. MADD's participation is contingent upon the availability of the requirements of your request. We never charge for these activities but voluntary contributions to MADD Northern Virginia are gratefully accepted.

Please fill out a new ‘Request Form' for each presentation you would like. For example, if you are requesting two separate speaking presentations on the same day, fill out a clean form for each request. If you would like to have a speaker and a booth display and the Crashed Car Trailer Exhibit, fill out one form for the speaker, another for the booth display, and a third for the Crashed Car Trailer Exhibit.

Requests for Public Awareness Events:
If you need only written materials for your event, please fill out the "Request For MADD Representative &/or Materials" form to reserve those materials for your event. We recommend making those reservations at least two weeks in advance, along with a scheduled date to pick up your materials; we will confirm material availability for you within the week. Depending on the nature and size of your event, we may also provide volunteers to staff the venue. If you would like a MADD representative to work the venue, please provide us with at least four weeks notice.

Requests for a Speaker:
If you need a speaker for your event, please fill out the "Request For MADD Representative &/or Materials" form. When you request a speaker or volunteer representative from MADD, please give us at least four weeks to coordinate your event. We will not contact you upon receipt of your request, unless we need to clarify information; our volunteer speakers will be notified of your request within the week. MADD will notify you as soon as a speaker volunteers to attend your event; if a speaker has not been found two weeks prior to your event, you have the option to reschedule your date, or find a speaker with another agency, if necessary.

Requests for Crashed Car Exhibit Trailer
If you want to use our Crashed Car Trailer Exhibit, please fill out the "Crashed Car Trailer Exhibit Request" form as early as possible. When you request the Crashed Car Trailer Exhibit, please give us at least four weeks (some requests come in up to six months in advance of the requested date) to coordinate your event. Upon receipt of the returned fax form, we will call you to discuss the details. MADD will notify you as soon as availability has been confirmed. If you are outside the Northern Virginia area, you may be given a "non-confirmed" reservation that would become "confirmed " three weeks prior to your event if activities within our service area do not conflict. All of MADD's volunteers are concerned members of the community, trained to raise public awareness about the crimes of driving under the influence and underage drinking. MADD's Speakers Bureau & Public Awareness Teams consist of volunteers who arrange to take time away from work to educate our community about the consequences of underage drinking, the consequences of driving while impaired, and MADD's services that are available free of charge to the community.

Although we hope that you can meet the time limits above, we will try to accommodate you even if we have less time than normal to prepare for your event so do not decide to forgo making a request because your event is sooner than our desired notice time. We have, on occasion, been able to accommodate requests with only a couple of day's notice. We appreciate the invitation for MADD to be present at your event and look forward to working with you!
